ACADEMIC  BLOCK CERTIFICATE

 

DIRECTIONS:

  1. Attach a Transcript. Write the points next to each course on the transcript.
  2. Write and circle total number of points at top of transcript.
  3. Return to Mr. Reibenschuh by Thursday, April 13, 2017.
  4. Students who qualify for an Academic Block Certificate do NOT NEED. To purchase a Block. They will be provided to those who apply and qualify for it.
